Tsumadori-san and Okusan, as she was called (basically a polite way of referring to someone else's wife) are responding to a question about the meaning of a small design on the napkin. The Japanese are very detail oriented, have amazing patience, and feel an obligation to satisfy any inquiry. Traditionally, napkins are small, single ply and not very absorbent. Being rather fastidious, there doesn't seem to be much need for the handful of paper napkins that usually accompany a western meal.
